Application

How to apply for membership:

1. Download the application to join the Society of Stukely Westcott Descendants of America and save it on your computer. The application is an eight-page editable Adobe Acrobat PDF form, which you can open using Adobe Acrobat Reader or a similar PDF reader. Save the document using with your name in the file name.
   1. Click SSWDA APPLICATION to download the application file
   2. When the application displays in your browser, click the DOWNLOAD or PRINT option.  You may need to move your mouse pointer to the area just above the border of the application for these menu options to appear.
2. Use the form to document your line of descent from Stukely Westcott, and optionally add information about your descendants and the military service of your ancestors.

   2. If you are a descendant of a SSWDA member, use the form to document your line to that person and enter his or her SSWDA member number.
   3. Alternatively, trace your line back to a known Stukely Westcott descendant as listed in Edna Jay Lewis, The Westcott Family Tree, Rutland, Vermont, 1999 and enter the individual’s index number and the page number where he or she is listed.

3. When you have completed the form email it to BWestcott2@gmail.com or print it and mail to the address below.  In either case you must mail  a $15 check (“SSWDA”) for the application/membership fee to:
